Collection of Hair Specimen for Hair Folicle Drug Test

Step 7 - Final steps in the specimen packaging

  • Verify Chain of Custody Form is completed
  • Insert the Hair Specimen Envelope in the Biohazard Specimen Bag (small pocket)
  • Take the top copy (white) and insert it into the Specimen Bag (large pocket)
  • Seal the Specimen Bag - it is ready to ship


Step 8 - Send Sealed Specimen Bag

  • Put sealed biohazard specimen collection bag into the preadressed envelope.
  • The specimen can be sent via U.S. Mail, FedEx, UPS or other courier service to our laboratory.
